Hematospermia, as the name suggests, is the presence of blood in the semen. The milky white semen turns pink or red, or contains blood streaks, and a large number of red blood cells can be seen under a microscope. People with this condition may experience mild perineal, rectal, and lower abdominal pain, or symptoms of urinary tract infection such as painful urination. Hematospermia occurs when there is inflammation in the seminal vesicles, adjacent organs such as the prostate, urinary tract, and rectum. Bacteria spread to the seminal vesicles, causing inflammation, swelling, congestion, and bleeding. Therefore, the most common cause of hematospermia is seminal vesiculitis. Treatment for hematospermia should include the following measures: (1) Abstaining from sexual activity. (2) Treating the inflammation with sulfonamides or antibiotics. (3) Massaging the seminal vesicles and prostate to expel the bacterial-containing fluid as early as possible to facilitate recovery. (4) Using traditional Chinese medicine. In the acute phase, damp-heat accumulation is predominant, and treatment should focus on clearing heat and resolving dampness, cooling the blood and stopping bleeding. The formula used is a modified Si Miao San (Atractylodes lancea, Phellodendron chinense, Achyranthes bidentata, Coix lacryma-jobi, Taraxacum mongolicum, Nelumbo nucifera rhizome, Imperata cylindrica root, Rehmannia glutinosa, Cirsium japonicum, Cirsium setosum). In the chronic phase, yin deficiency with exuberant fire is predominant, and treatment should focus on nourishing yin and reducing fire, cooling the blood and stopping bleeding. The formula used is Zhi Bai Di Huang Tang with the addition of Eclipta prostrata, Ligustrum lucidum, and Agrimonia pilosa. In the later stage, qi and blood deficiency is predominant, and treatment should focus on tonifying qi and blood and guiding blood back to its proper channels. The formula used is a modified Gui Pi Tang.
