This disease is mainly manifested during sexual intercourse, when orgasm occurs and ejaculation takes place, with paroxysmal stabbing, colic, and dull pain occurring in the penis, testicles, perineum, lower abdomen, and upper scrotum. If it is caused by seminal vesiculitis, hematospermia is common; if it is caused by prostatitis, prostate examination and digital rectal examination can help in diagnosis; if it is caused by urethritis, it may be accompanied by urinary frequency, urgency, and dysuria.

How does Western medicine treat this?

1. Cold stagnation in the meridians type: Symptoms include pain in the penis and upper scrotum during ejaculation, sometimes radiating to the lower abdomen, cold limbs, preference for warmth and pressure, pale tongue, white and moist coating, and deep and wiry pulse.

Treatment principle: Warm the meridians and dispel cold.

Prescription: Angelica sinensis 10g, Paeonia lactiflora 10g, Cinnamomum cassia 3g, Akebia trifoliata 6g, Asarum heterotropoides 3g, Evodia rutaecarpa 3g, Aconitum carmichaelii (decocted first) 6g, Zanthoxylum bungeanum 3g, Prunus mume 6g, Jujube 5 pieces, Glycyrrhiza uralensis (processed) 3g.

2. Liver Qi stagnation type: Symptoms include pain in the penis and base of the penis during intercourse, chest and rib distension, irritability, belching, poor appetite, pale tongue, thin coating, and wiry pulse.

Treatment principle: Soothe the liver and relieve depression, regulate qi and relieve pain.

3. Phlegm and Blood Stasis Obstruction Type: Symptoms include painful ejaculation, lower abdominal distension, perineal pain (especially after intercourse), difficulty urinating, white discharge after urine or defecation, dark red tongue, white and greasy tongue coating, and wiry or hesitant pulse.

Treatment principle: Eliminate phlegm and remove blood stasis.

Prescription: 10g fennel seeds, 10g cinnamon, 5g dried ginger, 10g angelica root, 10g chuanxiong rhizome, 10g red peony root, 10g poria cocos, 10g polyporus umbellatus, 15g alisma plantago-aquatica, 10g corydalis rhizome, 10g cattail pollen (wrapped), 10g trogopterus dung, 20g Liu Yi San (wrapped).

4. Damp-heat accumulation type: Symptoms include pain during intercourse, colic or dull pain in the penis, testicles, and perineum, irritability, bitter taste in the mouth, dampness or even swelling of the scrotum, dark yellow or dribbling urine, sticky and unsatisfactory bowel movements, red tongue, yellow and greasy coating, and slippery and rapid pulse.

Treatment principle: Clear heat and promote diuresis, disperse blood stasis and relieve pain.

Prescription: Gentian root 10g, Scutellaria baicalensis 15g, Gardenia jasminoides 10g, Plantago asiatica 10g, Akebia quinata 10g, Alisma plantago-aquatica 10g, Bupleurum chinense 10g, Angelica sinensis 15g, Rehmannia glutinosa 20g, Paeonia suffruticosa 15g, Achyranthes bidentata 10g, Taraxacum mongolicum 30g, Cirsium japonicum 30g, Glycyrrhiza uralensis 10g.

5. Kidney deficiency and essence depletion type: Symptoms include painful ejaculation, which is not severe and is mostly dull pain, accompanied by soreness and weakness of the lower back and knees, dizziness, general weakness, poor sleep, and often accompanied by low libido or weak erection. The tongue is pale or red, and the pulse is deep and thready or thready and rapid.

Treatment principle: Tonify the kidneys and replenish essence, regulate yin and yang.

Traditional Chinese medicine prescriptions for treating painful ejaculation

1. Take 2.4 grams of tortoise shell powder, 2.4 grams of turtle shell powder, and 1.5 grams of cinnabar, grind them into a fine powder, and take with rice wine.

2. Take 30 grams of Liu Yi San, 15 grams of Jin Qian Cao, and 15 grams of Hai Jin Sha, decoct in water and take one dose daily.

6. Take 60 grams of Paris polyphylla, 10 grams of safflower, 15 grams of prepared frankincense, and 15 grams of prepared myrrh. Decocted in water, the decoction is used to fumigate and wash the affected area twice a day.

7. Grind 0.5 grams of nux vomica into a fine powder, mix 0.3 grams of musk with sesame oil to form a paste, apply to the navel, and secure with adhesive tape or analgesic plaster.
